Hospital Data Collection and Reporting
TMF can assist hospitals with the process of data collection and submission to the QIO clinical data warehouse through My QualityNet using the CMS Abstraction & Reporting Tool (CART). Hospitals may also use a Joint Commission vendor for public reporting of core measures data for the Annual Payment Update.
QualityNet
Established by the Centers for Medicare & Medicaid Services (CMS), QualityNet provides healthcare quality improvement news, resources, and data reporting tools and applications used by healthcare providers and others.
CART
The CMS Abstraction & Reporting Tool (CART) is data collection software that providers can use to collect core measure data for submission to QualityNet.
HCAHPS
The intent of the Hospital-Consumer Assessment of Healthcare Providers and Systems (HCAHPS) initiative is to provide a standardized survey instrument and data collection methodology for measuring patients' perspectives on hospital care. HCAHPS data is reported on the Hospital Compare Web site.
30-Day Mortality Measures
To improve the quality of our nation’s hospitals, CMS developed health outcome measures for all hospitals. The risk-standardized, 30-day mortality measures are currently reported for acute myocardial infarction (AMI), heart failure (HF) and pneumonia (PN).

Sherri Gagner, Data Reporting Consultant at TMF also shared Data Reporting Updates, followed by a short Q&A session. This free Web-based presentation took place July 1, 2009.
